To access the e-HEFAMAA portal for registration or renewal, the under-listed documents are required:
A-NEW REGISTRATION:
- Letter of Intent to the Commissioner for Health, Lagos State.
- CAC Registration Certificate
- Letter of Introduction from LAWMA
- Stamped letter on monthly data rendition from the Medical officer of Health LGA (HMIS).
- LASRAA Card
- Inner Sketch Diagram. *
- Access the Diagram from Alausa to Facility Location. *
- Credentials of all Medical Staff
B-EXISTING FACILITIES
- CAC certificate (if accessing the portal for the first time)
- Tax Revenue Receipt
- LAWMA Medical Certificate
- Stamped letter on monthly data rendition from the Medical officer of Health LGA (HMIS).
- Last Registration / Renewal Certificate
- Current Practicing License of all Medical Staff
- Sworn Affidavit (in case of change of operating officer during the year)
Note: i. All * above are not applicable to Mobile Clinics, Ambulances and Home Care Services.
- It should be noted that if the Operating Officer or the Medical Officer is changed during the process of registration, the facility should write a notification letter to the Agency and attach the Sworn Affidavit and other relevant documents of the new operating officer.
iii. Health facilities should only commence operation after the inspection is done and the Agency gives approval.
- All Diagnostics, Laboratories and Eye Centers are exempted from HMIS clearance from the Local government.
Kindly follow the steps below for seamless navigation of the Portal
STEPS
- Type in the URL ( web address)
- Type the facility e-mail address with password on the page (login page) that pops up after typing the URL of the portal, this will take you to the user’s dashboard if you are already a user on the portal. Click on FACILITY MANAGEMENT and then on REGISTRATIONS, then click on the action button to start renewal entries and upload the required document. A staff complement template will also be downloaded and uploaded afterwards.
- If you are a new user, click “SIGN UP” this allows you to register and create a PROFILE. Once a profile account is successful, an automated email from the e-HEFAMAA portal will be sent to the email provided to verify and ascertain the authenticity of ownership. If the Activation link is not in your inbox, check the Spam or Junk mail. Your dashboard will display an overview of your transactions on the portal and from the menu panel on the left side of the screen.
- Click on FACILITY MANAGEMENT and then on REGISTRATIONS. This will display a page list of already created facilities if any. Click the green button “Register Facility” for a new registration on the page that opens up. A blank form will be generated for you to fill out accordingly. The required documents will be allowed for upload. A staff complement template will also be downloaded and uploaded afterwards.
- Click on “Next” at the bottom of each page till the end of the form at which point you click “Preview Registration.” A summary page is displayed, then click on” Submit Registration” with an automated invoice displayed on the computer screen.
- Print the invoice and make payment at any GTBank Branch with the appropriate item’s code (001 or 002) written on the teller given or collected from the Agency.
- Upload evidence/receipt of payment. (Please indicate the code of payment on the Teller before upload)

Please Note:
i. A Good internet service is required at every point of the process.
- All documents must be in PDF format 150kb
iii. There will be a penalty for the intentional upload of any invalid documents.
- Please, generate the Receipt on every transfer made and then upload it to serve as proof of payment for renewal/registration.
