How to Seamlessly Register Your Health Facility with HEFAMAA

Running a Hospital, Clinic, Maternity, Diagnostic Centre, Laboratory, or any health-related facility in Lagos State. One of the most important steps you can take to show professionalism and earn the trust of your patients is by registering your facility with HEFAMAA.

Not only is this registration required by Law, but it is also a big step toward providing safe, credible, and quality healthcare services.

Here is a Step by Step guide on the process of Registration/Renewal.

✅ Step 1: Gather Your Required Documents

Before you head to the e-HEFAMAA portal to register or renew your health facility, make sure you’ve gathered ALL necessary documents. Missing any of the listed may delay your approval.

📌 For New Registration, you need:

✍️ Letter of Intent addressed to the Special Adviser to the Lagos State Governor on Health.

🧾 Corporate Affairs Commission (CAC) Registration Certificate.

💰 Tax Revenue Receipt.

🧹 Letter of Introduction from LAWMA (waste management)

🏥 Letter of Introduction (stamped) from the Medical Officer of Health of your LGA (HMIS)

✅ Letter of Good Standing from your professional association

🆔 LASSRRA Card (Lagos State residency ID)

📐 Inner sketch layout of your facility

🗺️ Access diagram from HEFAMAA Office (Alausa) to your facility.

🧾Sworn Affidavit for the Operating Officer (In case the Proprietor is not a Medical Practitioner).

🎓 Credentials of all medical staff (merged into one PDF document):

• University Certificate
• MDCN/NMCN/RRBN Certificate
• Current Practicing License

And any other relevant qualifications.

🔍 Important Note: These documents must be scanned clearly and merged correctly for online submission. Errors can lead to delay in your registration process.

Once your documents are completed, visit the e-HEFAMAA portal via https://portal.hefamaaportal.com.ng/login to create an account and submit your application.

📝 Step 2: Generation of Invoice

• After the submission of your Application, an Invoice will be generated indicating the total amount to be paid, depending on the category (Scope of Service rendered, e.g., Hospital, Lab, Dental Clinic, etc).
• After payment to the designated GTBank Account details, the Payment Receipt or Teller will be merged with the generated invoice as one PDF document for upload in the payment management section of your e-HEFAMAA registration profile.
• Visit https://hefamaa.lagosstate.gov.ng/registration/ for renewal and registration fees details as applicable.

🔍 Step 3: Prepare for Physical Facility Inspection

After the upload of your proof of Payment and  Document Approvals, HEFAMAA will schedule an inspection of your health facility. The purpose of this visit is to verify your compliance with healthcare standards. Here is what the inspection team will check:

• Overall cleanliness and hygiene
• Availability of licensed health professionals
• Equipment and facility layout
• Waste management systems
• Documentation and records

⚠️ If your facility doesn’t meet the basic standards, you will receive guidance on what to fix before another inspection is scheduled.

🧾 Step 4: Get Your Provisional Approval

After the inspection and the criteria for registration are met, the Provisional Certificate of Registration will be generated. This means your facility is now recognized and can begin operating legally in Lagos State.

🔔 Note: This is only temporary. You must maintain standards and submit for full accreditation.

🔄 Step 5: Stay Compliant and Renew as Due

HEFAMAA conducts periodic monitoring visits. To keep your approval active, you must:

• Renew your registration annually. Registration starts early February every year.
• Keep staff licenses up to date
• Maintain cleanliness and proper records
• Continue adhering to medical guidelines

🚫 Non-compliance may lead to Sanctions, Suspension, or full withdrawal of your Certificate of Registration.
