Here is a Step by Step guide on the process of Registration/Renewal.
Before you head to the e-HEFAMAA portal to register or renew your health facility, make sure you’ve gathered ALL necessary documents. Missing any of the listed may delay your approval.
✍️ Letter of Intent addressed to the Special Adviser to the Lagos State Governor on Health.
🧾 Corporate Affairs Commission (CAC) Registration Certificate.
💰 Tax Revenue Receipt.
🧹 Letter of Introduction from LAWMA (waste management)
🏥 Letter of Introduction (stamped) from the Medical Officer of Health of your LGA (HMIS)
✅ Letter of Good Standing from your professional association
🆔 LASSRRA Card (Lagos State residency ID)
📐 Inner sketch layout of your facility
🗺️ Access diagram from HEFAMAA Office (Alausa) to your facility.
🧾Sworn Affidavit for the Operating Officer (In case the Proprietor is not a Medical Practitioner).
🎓 Credentials of all medical staff (merged into one PDF document):

🔍 Important Note: These documents must be scanned clearly and merged correctly for online submission. Errors can lead to delay in your registration process.
Once your documents are completed, visit the e-HEFAMAA portal via https://portal.hefamaaportal.com.ng/login to create an account and submit your application.
After the upload of your proof of Payment and Document Approvals, HEFAMAA will schedule an inspection of your health facility. The purpose of this visit is to verify your compliance with healthcare standards. Here is what the inspection team will check:
⚠️ If your facility doesn’t meet the basic standards, you will receive guidance on what to fix before another inspection is scheduled.
After the inspection and the criteria for registration are met, the Provisional Certificate of Registration will be generated. This means your facility is now recognized and can begin operating legally in Lagos State.

HEFAMAA conducts periodic monitoring visits. To keep your approval active, you must:
