The board of the Health Facility Monitoring and Accreditation Agency at its last meeting decided that due to the COVID-19 outbreak and the economic impact of same, the
- Increment in the registration renewal fee should be put on hold. Therefore, the Agency has reverted to the old fee. Health facilities that have paid the increased fee will be discounted for the year 2021 renewal.
- The deadline for the online renewal of registration be shifted to the 31st of May, 2020.
All health facilities are to visit the HEFAMAA website (www.hefamaa.lagosstate.gov.ng) for the guideline for the online renewal of registration.

A QUICK GUIDELINE TO REGISTRATION/RENEWAL OF HEALTH FACILITY ON THE e-HEFAMAA PLATFORM
Please note the following:
- Good internet access is essential at every point of the process.
- You are required to upload the under-listed documents (to be scanned in PDF format) to complete the registration process.
- Letter of Intent to the Commissioner for Health Lagos State ( for new health facilities only)
- CAC Registration Certificate
- Tax Revenue Receipt
- Letter of Introduction from LAWMA (for new health facilities only) / LAWMA Medical Certificate ( for old health facilities only)
- Letter of Good Standing from Professional Association or the regulatory body for the Operating Officer
- Stamped letter from LGA M&E Office ( for new health facililites only) / HMIS Clearance (old health facilities only)
- Last Regsitration / Renewal Certificate (old health facilities only)
- LASRAA Card
- Inner Sketch Diagram
- Access Diagram from Alausa to Facility Location.
- Staff List
- Credentials of all Medical Staff
- Sworn Affidavit (where the operating officer has changed)

STEPS
- Type in the URL (web address) hefamaa.lagosstate.gov.ng
- Click on “PORTAL” located at the top right corner of the home page
- Click on “New Profile Registration”.
- Supply correct information on the form that pops up.
- Please use a valid email address.
- Also provide a functional telephone number.
- Your password must be a minimum of 8 characters and must contain at least 1 uppercase letter, 1 number, and 1 special character.
- Once done with Profile Registration form, click on “Create Account”, the green button at the bottom, follow the link in the email. Enter the profile ID and Password (sent to you via email) in the space provided and click on “Sign In”. Your User Dashboard opens up.
- There is a navigation panel on the left side of the user dashboard. Click on “Facility Management” and then on “Facilities”.
- On the page that pops up, click on the green button “create new facility” if registering a new facility or on the red button “create existing facility” if registering an old facility.
- For an existing facilities key in your HEFAMAA facility code/Facility registration Number to continue. A form prefilled with your existing information is generated. For new facilities, a blank form is generated.
- Fill or update each form. Click on “Save & Continue” at the bottom, to move to the next form.
- Upload all required documents in their respective fields (PDF Format), save. and confirm the information.
- Generate an invoice for payment (New Facility) and make payment at any GTBank Branch /upload teller (Renewal).
- Upload evidence of payment (teller only)
